Hello St. Louis Area Community Members
St. Louis DPC practices are collaborating to offer an afternoon to receive your Flu and/or Covid vaccines at a convenient, quiet location in Brentwood.
We are working with Ladue Pharmacy to provide these vaccines. If you are interested, please fill out their secure form, which includes your health and insurance info, so that they can run your insurance and confirm coverage.
If your insurance doesn’t cover it, the cost would likely be between $50 and $150 per vaccine. But they will let you know prior to receiving, so you can make a decision.
In order to participate, you:
Must sign up by end of day on Oct. 19 by filling out the form for each person receiving vaccines, and
Show up on Oct. 21, between 1 and 3 pm.
More appointments will be added if the current slots fill up.
NOTE: this form will require you to choose an appointment time. If you end up not being able to make it at appointment time you scheduled, please feel free to come whenever you are available that afternoon.
